The new document does not require a notary. Just make sure to verify which one your consulate is using as it seems all consulates have not yet updated to the new form yet. I was heavily researched by the DOS and the documents cannot be found and it is not listed on any of the computers for NIV or IV services. US Embassy and Consulate Discussion	Packet 3 lost	well thinking we were making things easier, we went to the mumbai consulate and personally turned in Packet 3 information. We have been checking every couple days to find out when packet 4 was going to be sent and interview scheduled. Now today Mumbai consulate is telling us they did not receive the packet 3. Guess got to be patient and complete the forms again.
