IR-1 / CR-1 Spouse Visa Process & Procedures	Form DS-3026	We don't have our interview date yet (we have to wait until July 15th to schedule). When we had received our packet 3 from Naples, the ds 3026 had been included along with an attachment with instructions to fill in name, dob, and section 1 past medical history. I don't think filling out those parts would hurt at all, since they do not take more than a couple of minutes and then you will be sure you have everything when you go. IR-1 / CR-1 Spouse Visa Process & Procedures	Form DS-3026	Hi Barbara! We are also going through Naples (although for k1). Ds 3026 you only need to fill out part one, the past medical history part. It only takes a couple minutes I'm not sure about the vaccinations but with k1 there is another exam after.
