In a similar vein to what has already been posted, my fiancee is prepping to send back packet 3 (K-1), but we're a little stuck on what exactly to send back.  According to the checklist (http://photos.state....ket3-k1-mtl.pdf), she has to have the DS-230, DS-156, DS-156K, and the confirmation page for the DS-160.  

First question, do copies of all of those go with the checklist to mail back for the packet 3, or just the DS-160 confirmation page and the checklist itself?
Second, if the DS-156 is no longer available (we couldn't access it), is she only to bring the DS-160 to the interview?

Their information is definitely incorrect as I have never been married had not ever filed any of these forms.  There have been occasions where my father and I were mistaken for one-another on some credit card accounts, but nothing so serious as this.  Where do I find a contact number for the USCIS Tier 2?

Update:  I have found the contact number, will call on Monday when their live reps are available after I get off of work.

I filed an I-129F for my Canadian fiancee that was accepted on 2/21/13.  Received RFE on 7/16, hardcopy on 7/19.  I was expecting a request for more relationship evidence, but surprisingly it sated that their records indicated I had not only previously filed a I-129F that was approved in 2004, but that I had also filed three separate I-130's that were approved.  It requests a statement on the subject, but what can I possibly send regarding that.  At that point in time I was still in university and had just turned 21?

I received the RFE hardcopy on 7/19.  It says that on my original I-129F I stated that I had never filed an I-129F in the past and that their records indicate that I filed one for <unknown Japanese person's name> that was approved on 4/21/04.  It then requests that I make a statement regarding that relationship.  It then goes on to ask if I married her.  In addition it states that I have filed three I-130 forms  that were approved.  Of course my G-325 I indicated that I have never been married, and it asked for a statement regarding my marital status.  

I have never been married and until just last year had never seen or had a reason to file any of these forms.  In fact, in April 2004 I was one month shy of graduating from university in Iowa where I lived on campus for all four years after graduating high school.  

I am not entirely sure what kind of statement I can make about what they are requesting other than to flat out deny that I had ever done any of the things their "records" indicate.  Shouldn't my personal info and SSN be enough for them to determine that their records are erroneous?  I don't know what kind of documentation I can send to help clear that up.

Any suggestions would be greatly appreciated.
