Medisys takes verbal confirmation of VH history. If you had it they write VH on your vaccination sheet. Its one of the questions on the sheet they make you fill out " have you had chicken pox " check yes or no. I had mine done at medisys and i have had chicken pox, if you havent had it they will ' likely ' give you the shot but they might not too. I know that shingles is horrible to have and it can be very dangerous when you are older. Medisys will give you any shots that you are missing but it will be an extra charge. They gave me my gardasil there and it was the only one I was missing. I had shingles already at a young age which is rare, usually only elderly people get shingles! ( shingles happens after you had chicken pox ) You have to pick up the medical as far as I know unless they changed it. I called and was told they don't deliver to the embassy that you have to pick it up but that they can hold it till your interview so you can pick it up the day before if you want instead. wrote that i had it ... but i did had it ! lol so i should not worry... the real one i was expecting to get was Tetanus * not sure if its the good word but its a shot that protect you from metal stuff... anyway she didn't mention it at all	Persona	Female	Canada	2009-11-21 00:03:00

Canada	MEDISYS Medical Montreal	First off to go there you take the metro to MCgill then you go out on Union street and walk straight up the hill.. you turn right on sherbrooke and you will find the building one street after. As soon i enter the place i felt good about it.. I was damn stressed but everything was decorated for Christmas and it didn't look like an hospital at least... i took the elevator to the 11th floor and when inside their waiting room. A sign tell you to wait and someone come at the window and tell you to come. They asked what i was there for and requested to see all those *P4 *vaccination record *passport *3 picture passport size then she made me go sit.. and not even 3 min after a lady called my name... she was the nurse and she was really nice I went in a room with her and she took my pressure and my blood in 2 vials... i told her about gardasil and that i didn't wanted to take it... she said all what she do was to put date of my older vaccine and left that one blank so she didn't wrote refuse or anything.. then she took my weight and let me go back to the waiting room. she mentioned to talk to the doctor about it. she also check my eyes and said i had 20/20 even though i pretty much guessed both line she wanted me to read cause it was so blurry waited 3 min ...was really surprise how fast it was going... now it was the turn of the X ray got called and another lady told me to undress from head to waist and put the jacket.. then she asked me if i was pregnant to which i answered i didn't knew for sure but the chance would be probably real low.. so she didn't took chance and gave me a heavy thing to wear on my back for when she did he x ray... took 5 min and it was done ...she made me place myself on weird positions.. she was also super nice and she said my picture where fine and looking good ...she sent me back to waiting room...( dressed back of course XD) waited 5 min and the doctor called me... ( DR Pigeon ) he did my medical real quick... check if i didn't had visible deformation im guessing ... he made me undress in panties and bra and had to wear that horrible blue jacket again.. took my heartbeat check my mouth and ears and that was it... told him about gardasil and he was not really cooperative ,,he kept telling me that it wasn't is job that he didn't had anything to write on my folder basically and sent me back to waiting room paid and got a receipt.. telling to call them in 3 days to get my papers... if you got more question ill be happy to answer... So outside of taking it with you to the medical, and perhaps bringing it along to the interview for sanity's sake, I can't think of any reason they'd need a copy of it. If you pass the medical, they'll know you satisfy their requirements. All of us more recent P3 filers sent in our forms. The woman who was processing us had all of my forms in front of her, and waved me away when I offered to give her mine. However, it's always a good idea to bring copies, if you can spare the few bucks it might cost to make copies of everything. For myself, it's worth it to not go crazy with worry if they happen to ask for something I don't have in front of me. ya my medical won't be good since i haven't took gardasil so im bringing the original and ill photocopy all the book in case... Mine's just a little yellow booklet, which basically fits on one page when I unfold it. That doesn't make sense, but either way. lol You could try taking it to a local immunization clinic, ask them to update their records to match yours (if they're not already the same), and they should be able to print something for you. That's what I did for mine when I got my most recent shots done. good idea but ya mine is a book with 20 pages ...i have 3 pages of vaccination about the size of 15 CM by 6 CM so its kinda annoying to print that XD and i wonder if medisys would be nice enough to do it for me when i go see them monday... Canada	In preparation For Interview K-1 MTL	alright so im following the checklist from http://www.visajourn...h...25594&st=30 and now my question is 1- i think i might have sent the p3 (checklist page) with the p3.. i can't find it... what can i do? 2-i am trying to find how the hell do i make a copy of my immunization reccord? do i take my book and scan and print all my pages ? cause there is a lot! i think that's all what im looking for at the moment... will probably add more in the next few hours or tommorrow since me and persona will be looking through everything again... also since i had the new P3 i sent all those papers already...what should i do ? reprint in case they ask for those again ? Canada	InterView K-1 Montreal	It was 6 when we left our hotel room in that cold day...(minus 2 degres celcius) Our hotel was really close from the consulate...about 5 mins http://www.hotelcent...om/index_fr.asp it was nice and cost less than 200$ for 2 night... (ill make a post later about it with picture of our room) as we arrived there we saw 2 other members of VJ ... but since they don't troll the forum i can't say their names... the security open the door just about 15 min before 8 am and there was about 10 people waiting with us... we entered 3rd and went through the security i will skip cause that part is boring and basicly resume into..(waiting!) around 8 am someone called us at booth 9 and something people forgot to mention or i just probably never read it... YOU Will have to stand the whole time at a window.. and even in the interview there is no chair to sit ...lol not that it matters but still it surprised me.. We were really prepared and everything went fast as the guy asking us for paper was getting what we wanted asap from us... i can't remember what he asked us in a proper order but ill edit the post later if i can then he sent us to wait again.. when i got called again it was to do the fingerprints and give me a paper to go pay the interview fee.. *went back to wait again* he called us again... said everything was in order gave me back the medical on CD from medisys (x ray are on it ) and he sent us to wait..... Again..lol at that time it took around 1 hour the interview++ We then got called to a room where a nice guy was behind the glass of course greeting us with a smile.. then we had all the question on how we met and personal stuff he checked our paper and of course my worry went on the table when i saw a piece of paper in front of him wrote in red ink *DEnial HVP* Persona was holding my hand as we waited for the answer. *he said.. you haven't took the vaccine but.. we do know the law will change and by the time of your Aos this will be removed...* i think when he said that... we both felt all the pressure and stress got out our shoulders, i was really relieved he asked a couple more question saw the picture persona sent to them and got angry at him for sending one of me at universal studio where i was in an akward position having my #### eated by the giant shark.... XD i said *You're gonna hear about it!!!* the interviewer said ...well hes hearing it now.. we laughed i replied he would keep hearing it..for years... he looked at persona and ask him if he still wanted to marry me ..that he could still not send the papers .. persona said something like no no its fine lol and he approved us Thats it... we got really lucky i think that the timeline happened to make me not take that evil shot... I'm not at all accusing YOU of cheating the system--cuz you're not. It's possible to be filing taxes for both countries and still maintain residential ties for tax purposes with Canada because sometimes it's difficult or not convenient to sever them right now. Everyone has their own reasons. Depending on the situation, it might not be a bad idea. I'm just highlighting the real risk that if one filed with the CRA that they are non-resident, make sure they really meet the requirements of non-resident from the CRA's perspective. From the tax cases I studied, one guy who severed all ties with Canada, but had a canadian driver license, got nailed as resident. He had to pay taxes on his worldwide income for some 3-4 yrs. Another guy, also severed all ties with Canada completely, but his wife (unknown to him) applied for Canadian child benefits and received them. That was enough to deem him a resident, and again had to pay taxes in the area of $400K! I studied cases closely because my tax situation is very complex and I wanted to make sure I dotted all my i's and crossed my t's when I finally move to the US. im confuse... lol... does it mean next year ill have to fill 2 still ? like this year i fill just one ? or do i need to fill one in the US as well since ill be staying since 2 months ? Notify Canada Revenue Agency of your new US address. They will mail you the appropriate tax return for the Province you left early in the new year. You would file exactly the same way as you did in Canada but you would only file up until the date you left. Don't worry about getting an International Driver's License - your Canadian license will be fine in the US in most states for the first 30 days. Keep a Canadian bank account open and set it up for internet access. Provide Canada Revenue Agency with direct deposit information for that account. They can deposit your return to that account and you can then access the information via online.
