Hi, I'm not sure if I should be posting this here, or in the DCF forum, as my spouse and I are applying though DCF in Japan. Please let me know if I should move this.

Anyway, I have a couple general questions about filling out the g-325. My first question actually relates to all of my forms since I have yet to change my name on any of my documentation. However, I plan on changing my name once we return to the states. Should I file my forms using my married name, and show our marriage certificate as proof of name change, or should I use my maiden name so it will match my name on my passport, SSN, etc.?

Next question is about needing full names and addresses of employers for the last 5 years. Are the full addresses of employers needed? Or would stating city and country/city and state be enough? There isn't enough space on the pdf form, so I'm not sure if I need to create a separate attachment to add that information.

On the bottom of the form I'm supposed to check that this form is accompanying an application for "naturalization" "status as permanent resident" or "other." I'm submitting this form in conjunction with the I-130 petition, so do I check "status as permanent resident" or do I check "other" and write I-130 petition?

Last question: At the very end of the form it says, "Applicant: Print your name and alien registration number in the box..." Do I put my husband's name (I'm the US citizen) on both of the forms and put "none" in the AR number box since he doesn't have one? Or do I put my name and SSN in the AR number box on my g-325, and his name and "none" in the AR number box on his form?

Sorry for asking so many #######-retentive questions about such a simple form, but I really don't want to make any stupid mistakes. Thanks in advance for any help or advice!
