PhilippinesWhy use TB SPUTUM test and not the TB Skin test?
QUOTE (EricN @ Dec 2 2009, 11:22 AM) <{POST_SNAPBACK}>
Actually the blood test is the best combination of accuracy and quick results. It is what my infectous disease doctor used on me here in the States to determine I was negative. (He by the way is from PH as well!) However, the 3rd world countries cannot afford this method.

I believe the Spetum also gives false positives.

The skin test is accurate, especially for those in the province never immunized to begin with.

My wife tested positive on one of 3 spetum tests. I asked for validation via the blood test, they refused. 6 of her family members living with her all tested negative including her daughter that sleeps beside her. Now we have been delayed what will be a total of 9 months from the day they notified us until the day we will finally get a visa. It sucks esp. for those not from Manila who have to find and pay for a new place to live in Manila.

Just wondering if anyone know the answer...

TB Sputum test culture is 2 months...for the results

While the TB skin test you can see if you have been exposed to TB in just 2 days......

Other then SLEC trying to make few extra dollars from the poor souls.....why use one over the other?

maybe some one in the medical field can explain.....

Thanks

here is some info from CDC

http://www.cdc.gov/t...ing/default.htm

If I read it correctly it have the Skin test to see if been exposed..... and then the Sputum culture to Diagnosis of TB Disease

Persons suspected of having TB disease should be referred for a medical evaluation, which should include a

- Medical history,
- Physical examination,
- Test for TB infection (TB skin test or special blood test),
- Chest radiograph (X-ray), and
- Appropriate bacteriologic or histologic examinations (tests to see if TB bacteria are in the sputum).

Simply because a TB sputum culture is basically the gold standard of determining definite TB activity. The PPD or skin test will give a false positive results, given that all Pinoys get a BCG in infancy.

OK...sound like a decent answer but anyhow, it is a time killer for those unfortunate ones that show problems in the XRAYs.....

yes, BCG will cause a false positive PPD, unless you are one of the rare ones who's immune system never sensitized to the vaccine.. and the TB culture, by standard, is read as "negative" if nothing grows by 42 days... i would assume that any, and i mean any abnormality seen in the lung fields on CXR, would be a knee-jerk reaction to order a sputum culture.. this is because someone with TB that is inactive, may show tiny scarring or nodules. the last thing we need right now is to allow TB carriers into the country , untreated, which would invariably result in a significant rise in hospitalized patients with TB. i pay enuff taxes! let's not give uncle Sam another reason to hike them further. ;p as a side note, in my 6 short years of medicine, i have NEVER managed a patient with known TB. most docs here have to look up how to treat it if they come across it, because they rarely ever see it. however, my older brother went to medical school in PI, and saw it ALL THE TIME as a student. i think that speaks for itself, and says we're doing something right.......... of course, if they throw that sputum culture at MY fiance when it is time, I'll end up throwing a FIT!!! haha ;p

the thing about the TB bacillus is that it grows really slow. Yes, it sucks but nothing can be done to speed it up. The quantiferron test is the blood test done...yup, it's really expensive, but most accurate.

PhilippinesITIN -- certified copies
before i arrived, hubby and i tried to apply for ITIN so he could file his 2008 taxes. the form for myself and my daughter called for our passport numbers among other documents. since our passports were issued by DFA, i had photocopies of them certified there with a red ribbon affixed to the certificate (and it does look like a certificate). paid Php120 i think to have them delivered via courier to my house (because i found it a pain in the a-- to have to go back there a 3rd time). All was for naught...don't recall now`why but hubby decided to ask for an extension till we got our SSN's to file his tax returns.

PhilippinesInterview June 4th for my wife and child

My wife and my stepchild are supposed to be in Manila on June 4th for there interviews. I would like to get a idea on what kind of questions they are asking. I went over the Wiki search engine for some answers, and if they start asking some of the questions on that list, my wife is in trouble. There is no way she is going to remember my ex-wife's name, where I worked and did for a living, what my daughter's name is, ect.. I had a very nasty divorce right before my new wife got married, so there wasn't a lot of talk about my ex-wife. My wife's english is not very good, and I think she should probably ask for a interpeter to assist her so she can better understand the questions. She will not be able to answer questions like, how much child support I have to pay, alimony, ect..

Also, the only proof that we have for our marriage is the phone bills that I saved with here phone number on it, wedding photos, and all the Western Union receipts that I have saved for money that I sent to her. Also one more thing, how about a cenomar? Why would I need that if she is married? I have NSO copies of marriage certificates and birth certificates.

Do I have something to worry about here? Are they really going to be asking all these questions?
thanks,
martinbr

Hi. Hope this link helps...details of my interview.

http://www.visajourn...Dec 9 interview

I was trying to find th e list of documents that I had posted on here shortly before my interview, but I have forgotten what the exact title was. But, anyway, other proofs of your relationships would be bank accounts, wedding pictures and pictures of you together with your wife and pictures with your stepchild. Tell her to make a separate collection of photos of herself with her child, to prove that the child has been with her since birth. Telephone correspondences, cards/letters to each other, emails, chat logs...they asked all of these from me. How old is your child? the only question they asked my then 6-year-old daughter was her name, my name, and her now daddy's name. Frankly, the pre-screener is more brutal than the CO  , and they seem temperamental for some reason. You may want to give her the answers to the questions ont he WIKI search engine and have her go through them every single day prior to her interview. She can be asked anything.

About the CEMAR...your wife will be filling out the same form as the CENOMAR. We had submitted one with our papers but I requested my own copy to show the CO if asked for it.

Goodluck to you all :)

PhilippinesChange child's surname to mom's maidenname

Ok, I did a little searching and you cannot change a stepchild's name (K-2 visa) through just the marriage of the K-1 to the USC. The USC must legally adopt the child if you want the child to have the last name of the USC. This is consistent with the laws for step children already living here in the U.S..

Edit to Add: For Rhiz - I would think that as long as you can legally change his surname in the Philippines, when you finally petition him, you would just need to show immigration documentation of his name change. As far I understand - under Philippine Law, mothers of children born out of wedlock have sole legal rights to the child.

you'll also need some document from the father that he is relinquishing his rights to the child,an that he is consenting to you bringing him here to the US, since his name appears on the birth certificate

PhilippinesStep Parent Adoption
Have hubby petition your child as IR, then proceed with adoption here. It will take eons if he is to adopt the child in the Philippines as your hubby will need to stay in RP for at least 6 months. We are currently awaiting my daughter's adoption hearing here. STarted the papertrail last September. Lawyer bungled the paperwork. After Child Services did their home-study, we learned that we could've done the adoption process here on our own without a lawyer  Could've been done with the adoption process by now if we did

Philippinesi am a GC holder and i plan to let my parents visit in US

hi everyone..im a GC holder and my hubby and I wish to let my parents in the Phils. come here to the States as tourists since im not a US citizen yet to sponsor them..my question is will they need a complete medical at St. Lukes? or are they exempted?

thanks everyone//

daisy

no medical needed. They will need to apply for tourist visas on their own. The only thing they need to do is show/prove to the CO that they will not be of any burden to the US, that they have funds to support their travel and stay, and that they have a strong reason for returning to the RP. To the US, any person applying for a visa has intentions of migrating unless they can prove otherwise.

PhilippinesWife is on the way back home to get Visa

Wife is on the way back home to get her visa since she passed interview. It is waiting in Negros Occidental in which Air 21 tried to deliver once already. But my wife was still in Manila at the time. I tell you, I am not very happy with the Manila Embassy. Why they didn't wait on her interview and re-schedule at that time since my stepchild had some medical issues is beyond me. There is no way in hindsight that I would have thought to ask my wife to take this responsibility. Because that is what I was told by a official at the embassy. That they have no control over issues like this and basically it is really up to us to try to re-schedule. My wife did not speak up at the time because she either wasn't thinking or she just did not know. I tried to be as thorough as I could about this whole process, but this one slipped passed me. I did not think about it either.

So now the clock starts ticking on my wife's visa while my stepchild undgoes medication to clear up her lungs for the next time she goes to St. Lukes. Most likely in about two months. My wife originally told me six months, but found out that is in severe cases and two months for mild cases.

But if it goes past six months, my wife's visa will expire and I will have to request a replacement visa in which will cost me another $400.00 + the medical exam for my step child.

So I thought our journey was coming to a end here, but it looks like it's going to go on for sometime. I am sure I will get some flames here, but this is the way I see the situation in which occured in our immigration process.
martinbr

Sorry to hear about these issues. but take heart in that your wife WILL definitely be with you soon as will your stepchild.

Let her take some lonzenges to ease her throat. In fact any menthol candy will do. The Snow Bear is actually IMO better than lozenges. Also, antibiotics are not recommended as this stage as your daughter may develop resistance to the antibiotics in time.

Lastly, I don't think that her coughs would be an issue. The doctors in SLEC would be know what's best for your daughter.

Hoping that your medical will go through a breeze. smile.gif

I took a look at her throat. Children do not develop tonsils till they turn 7. In her case, her tonsil "buds" on the right has what looks like an exudate on the upper pole...may look like a canker sore or stomatitis ("singaw") to some. It irritates her throat. The cough prep is nothing more than Ascof syrup which seems to help her somewhat. She is rarely sick and is rarely on antibiotics. Ordinarily, I would not give her any, but the exudate bothers me.

Canker sores aren't viral so that should not be a problem IMO. Just avoid her eating spicy or sour foods as the usual remedy goes.

If you're still uncomfortable, you may wish to move your medical to a later date to easen up your worries.

BTW, I believe you are a doctor? Please correct me if I'm wrong. Just asking. smile.gif

Yes sir, I am. With a devil-may-care attitude in med school and residency, currently with an immigrant visa-induced case of OCD laughing.gif Am just wondering of what impact it may have on her medical. I guess if they ask to have her X-rayed, it would not be a problem.

Let her take vit. C. If you can find Fern C then it's good.

Police clearance are good for 6 months only. NBI is a year if im not mistaken. He can go to a station near him and he just have to pay an amount and will need a cedula (community tax certificate) and have his photo taken and he will be able to get it within an hour depending on the station. They will assist him there when he go there and usually they put the instructions in a big board in front of the station for everyone to read. good.gif

And do not forget to apply for a CENMAR (same application form as for a CENOMAR, only this lists that there is a marriage between the two of you)...this is the marriage index. Also, I woud suggest getting new NSO copies of your birth certificates and marriage certificates as these expire 6 months from date of issue. Gather all proofs of relationship...the more the better.

My daughter was having medication for primary complex when she had her medical exam at SLEC. I think this was the reason why she did not anymore have the tuberculin test and instead just had her chest x-ray which per her medical report from SLEC however, was said to be normal. Nonetheless, because SLEC noted her medication, her visa had the annotation about TB infection. When we got here in the US, we were referred to a pedia-pulmonologist just for her health records to be cleared/clarified. Thankfully, she's found clear of any TB infection that she is now on her last week of prophylaxis.

Don't worry much, your child will be cleared for sure. Good luck on your interview!

My infectious medicine is a bit rusty but 5mm is ok, 7-10 is questionable, 15mm is positive. My daughter's was 14mm and she was labeled positive. The gold standard for determining PTB still remains the presence of the tuberculosis bacilli on sputum culture. But even witht he proper environment and culture media, it is still hard to grow the bacteria.

Thing about primary complex, the treatment is differnt from one with active TB. Also, kids with primary complex can't infect other kids. They got the infection from an infected adult, apparently. Unlke an adult who is usually put on sick leave for at least 2 weeks upon starting treatment, kids go on ahead with their (less) busy lives and continue going to school. but anyway, would i have to repeat my medical after 6 months too?

Arrived at SLEC just before 3pm, appointment letters checked by the guard ( he checked both the photocopy and the originals) and we were given forms to fill out. No, I did not need to get any number. So...filled up the forms (NOTE: You will be asked for your nearest US relative...not your husband nor fiancee...if none, leave it blank. But you need to know your relative's address and phone number), gave them to reception who checked if I filled them out right then had our digital photos taken. Oh...affix one 2x2 picture to the form. And PLEASE make sure that you have the correct size and style photo...like the ones you put on your visa application forms. Anyway, I was directed to the 5th floor to pay the fees (Php10, 0667.??) then on to the 2nd floor for immunization interview. My daughter did not have any history of primary complex and was thus given a skin test. I did not get any. From there, on to the 3rd floor for my X-ray to 4th floor for my laboratory (urine and HIV) then to the 5th floor for the vital signs and physical exam. Bring any prior medical record if you have one. And you will be asked to strip down to your birthday suit..there is a gown you can put on, of course. We were asked to come back today, November 28 for the skin test interpretation. So we whiled away Thanksgiving in the mall. Today, we went to SLEC at 2:30pm and the guard checked our names against a list. He found my name and said that I can have my vaccines already, that I had passed (thank you Lord!). We had to wait till 4pm, however for the skin test interpretation. While waiting, I got my vaccines (MMR --- I had been vaccinated just last year but was unable to obtain a certification--- and TdaP), one on each arm....ouchy! Soon after that, my child's TST was due and it was read positive so that she was sent for an X-ray PA and lateral views. We were told to return on Tuesday, Dec 2 for her results. unsure.gif Thing is, my results will not be released without hers. And we have a Dec 9 interview sched.

What happens after this? What if her CXR comes out positive? What will happen next? Will we be delayed? What about my results? How long is it valid?

PhilippinesFinally done at SLEC
Thank you guys. I am relieved much, also knowing that our little girl is healthy. Met sasha414 and, well, I wish her all the luck as well.

Onwards to USEM....and hopefully a blessed holiday innocent.gif
