Positive PPD test means you were exposed to a person with Tb, in which the Philippines is endemic with. Exposure wouldn't mean you have Tb disease unless proven otherwise with the gold standard for its diagosis which is the Tb culture. PPD test is not interchangeable with Quantiferon Tb Gold Test. Quantiferon has just been approved by USFDA to check for Tb suspect patients. Negative Quantiferon means that you will be unlikely to have TB disease. It is not yet widely used in practice because there are still only few data to support its use.

False positive PPD result is seen in patients with kidney failure, diabetes , health care workers and those who receive BCG vaccine. Many people who were born outside the United States may have had a vaccine "BCG," which can lead to a false-positive test result. However, many experts believe that a past BCG vaccination should not change the PPD result when the test is done 15 years after the vaccination.



dbears, it is not yet recommended for use in persons younger than 17 years of age, as there are only limited data regarding its use. It is still a new diagnosic test.

Most of the Filipinos are exposed to Tb and most of the time, PPD will be positive also. The diagnosis of Tb in adults is not dependent on PPD or chest xray but rather on the detection of the Tb bacilli in her sputum. The gold standard for the diagnosis of Tb is the sputum culture, which takes about 2 months. Positive chest xray will not warrant anti-Tb therapy unless, she has an active moderate or far advanced Tb. But still, Tb culture should be done.

PPD or TST is sensitive in detecting exposure from Tb carrier or from those with the disease but it is not the predictive diagnostic tool.
Chest xray will show, Tb scars or cavitations (indicating active Tb). If the infiltrates are minimal, it is best to do sputum AFB stain and culture right away to identify the bacilli. Diagnosis of Tb using chest xray and TST will more likely result in overdiganosis, thus giving the patient anti Tb drugs when it is not needed.

I suggest, for your friend's peace of mind, that she consult a pulmonologist. That way, she will have the right answers she is looking for, provided she knows what questions to ask the doctor. If she indeed has Tb disease or not, then she will be treated accordingly.

if not what should i do now...i have to file my aos rite? and i need my medical report fr surgeon/ is there any reason that he will hold my medical bcoz my skin test tb is possitive? If your TST is positive, the civil surgeon will request for a chest xray and/or sputum culture and AFB stain.

what happen to me now? how will my aos goes ?pls tell me guys. You can still apply for AOS, if ever, you will have to undergo 6 months therapy at home. They may even restrict you from going places in the fear of spreading the Tb bacilli. Don't worry much, all will be fine. Just live each day one at a time. No worries.

Hi there Tina..Here is my 2 cents worth. As you have said, SLEC is quite strict with PTb screening. The doctors at SLEC and the doctors at St Lukes Hospital are not one and the same. Every clini has different SOP's. So, they may or may not accept the results of your Tb cultures done outside SLEC.

You may undergo chest xray, AFB smear stains and Tb cultures, months prior to your medical exam at SLEC. You will do this for you peace of mind and you'll have a heads up of what will come out of your exam.

Tb scars will disappear, but it will take a very long time. Maybe even years. At SLEC even if your AFB stains are negative, they will request a culture if they will see a scar on your xray.
