CanadaThings needed to send back with Packet 3
Inky already posted everything that needs to be sent back with Packet 3. Packet 3 is a little confusing because one of the forms tells you to attach your birth certificate, police check, etc. but that is NOT the case. You ONLY send the forms that Inky listed plus two american style passport photos and a photocopy of the biographic information page from your passport. The checklist also addresses both K1s and CR1s so not everything on the checklist is required of you (most K1s don't need to bring the 864 for example, instead K1s do the I-134).

You DO NEED to submit the 156K and 156, as well as all the other forms Inky listed. A lot of other consulates don't require the same forms, that is why you see conflicting information, but the Montreal consulate requires the ones that Inky listed (you can view it on the Montreal consulate website as well.) Remember, USCIS only sets the minimum for what the consulates must require from you, but individual consulates can ask for additional stuff, so the procedure for every consulate is different! That is why its important to check the individual guidelines for Montreal, and to take advice from those with experience from Montreal.

The police certificate is valid for one year. I would suggest going to your local police station and asking about how to obtain one (an RCMP fingerprint check is not necessary unless you've been convicted of a crime) and how long it takes. For the medical you need to obtain your vaccination history. You can read about which vaccines are necessary here http://www.panelphys...munizations.htm You only need to get a chickenpox (varicella) vaccine if you didn't get chickenpox as a kid. If you don't have your complete vaccination records you should find them ASAP.

For HIV, they ask you verbally if you have it and they also take some blood samples from you. I don't know what they test with the blood samples but if you don't have HIV it's nothing to worry about because you just tell them you don't have it. :)

It's possible but as everyone else said it's up to the border patrol officer. No matter what you do, don't lie! Remember nothing is there to stop them from leafing through your passport and seeing the visa there. So if you're asked just be completely honest and explain you don't want to activate your visa right away and have proof of ties to Canada, to show that you're not going to start living in the USA without a visa. You can try visiting after your visa's been issued but be prepared in case they turn you around. Is it not possible for your american fiance to come visit you instead? That would be much less iffy than you coming to visit him.

They actually don't care about the chest x-ray, I've never heard of anyone being asked for it at their interview. I brought mine nonetheless, and when I asked if they wanted it in addition to the medical exam it was a very curt "no." I think they must tell people the same thing every day! :lol:

Yep, that's more than enough :D That pretty much parallels everything I brought to my interview. They only asked for the bare minimum in my case, but better to be prepared in case they ask for something. The 864 is very rarely asked for from K1s at their interview- most people only bring the I-134. (I *think* the 864 is only requested if you can't prove your sponsor makes enough money to support you, or something...) Indeed the 864 usually isn't filled out until AOS.

Montreal did not ask for it from me either. I'd suggest printing out just a couple e-mails since photos and boarding passes are much better evidence than e-mails anyways. That way you'll have it just in case and ease your mind but you won't go crazy with the printer ink. It is more important to be familiar with your husband (lol), get ready to answers lots of questions about who, when, where, and how. I imagine they don't ask for evidence of a relationship unless they're doubting its authenticity, so if you seem genuine and answer there questions correctly you should be fine. :)

I don't have my packet 4 handy but I am pretty sure that if a K1 pays the fee at the interview it has to be in USD- CAD is not acceptable. It should be possible to pay the fee online after scheduling an interview but I am unsure how.

well I don't actually blame you for becoming a paper hoarder :), I wouldn't throw stuff away either until I know for a fact it won't be needed down the road. Speaking of which, I hope your x ray won't be needed for your AOS when they ask for the white immunization document, I'm sure you can still probably use it eventhough you've folded it? Hopefully it won't come down to it! But I think I agree, I was planning on leaving it in the car as I'm driving to Montreal, but just in case I will do the same, stick it into a back pack or something along with other documents!


I don't think it's needed for AOS actually :) Also the consulate won't let you bring a backpack inside. No large purses or bags or anything... No electronics at all either. I put my papers inside an accordion folder and put that folder inside a plastic grocery bag :) Just thought I'd give you a heads up in case that was your intention! lol

I'm like... 99% sure that Montreal doesn't care about the x-rays, but I brought mine just in case. You never know! They didn't ask for mine, though. I folded and mauled it in the process to make it a little less cumbersome. I would definitely bring it, because it's better to be overprepared.

This process turned me into a paper hoarder. That is, I save everything that I think might be possibly useful even if there's only a teeny chance that I'll need it. :rolleyes: I still have my x-ray and I'll probably still have it until I've done ROC even though I'm pretty sure it's not needed for anything :lol:

Okay, I did a little searching on the Canada forums here and it seems like it's okay. :)

Hi Brenda, The form DS-2053 is the Vaccination Documentation Worksheet they should of given to you from your medical, it has the list of all your vaccinations with dates on it. If you don't have it, maybe you can try calling Toronto and see if you can get a copy of that sheet. If not.. i'm afraid you might have to do it all over again :( but i'm not sure. I just know you have to send that Vaccination Document worksheet in with your AOS paperwork.

I sent in the Vaccination Documentation worksheet, the receipt from when I got the medical done and a letter stating i had the medical done within the year with the address and phone number of the office I had it done at and it was all groovy.

I hope you find your medical paper :)

Where did you do your medical again? When you picked it up from the office or received it, there should have been a page for your record. It's a copy of the immuzination sheet that the doctor put in the sealed enveloppe. That's the DS-3025.
Also, you can put in the cover page of your AOS package that you had a medical done within the last year and are therefore not including the I-693. This is what most of us August crew people did, and so far nobody received an RFE because of it...

It's funny, I could've sworn my sheet had DS-3025 written on it but looking at it right now it doesn't. Anyways, it should look something like this http://media.photobu...LISH_Page_1.jpg Even if it's not labeled DS-3025 it's fine :yes: You will not have to call the panel physicians to fill out another form or anything.

As for AOS, I'm pretty sure this sheet is still acceptable but I'm unsure. I think it depends on how ####### the case worker at USCIS is about it, because technically I think they want you to get the vaccination record transcribed by a civil surgeon in the USA but most of them won't do it without re doing the whole medical (with the hefty fee included) which is unnecessary if your medical is less than a year old. So it's something that a lot of people don't want to bother with. Not sure how often it causes problems, but even so it's not something you can do anything about until you're in the USA.

You can check out the AOS forums for more info, because when you're doing AOS it's back to USCIS which means everything is standardized no matter which country you emigrated from. :)

99% of people get the "blanket waivers" box checked because many of the vaccines (rotavirus, for example) are not age appropriate (they're meant either for babies or for those over age 65). I haven't done AOS yet (still saving up for it) but I'm pretty sure it's okay :) It shouldn't cause problems at the interview.

The vaccination sheet is also called the DS-3025 (it should say that in the corner somewhere), which is handy to know when you fill out your AOS forms.

Regarding the vaccines you probably don't have:

pertussis: is only covered for 15 year olds (in Ontario)
Hep a: is for travel to countries where you're more likely to be infected by it (Mexico, etc.) and it's not covered (in Ontario)
influenza: seasonal
influenza type b/hib: is for babies
pneumococcal: is for 65 year olds
rotavirus: is for babies

Yes, you should be okay. Generally if something is "wrong" they will require further testing. For example, I believe one VJ member on here had a lump on her breast and before they would complete her medical they required her to get a specialist to look at it to confirm it wasn't cancer. After she had done that she got her medical and it was all okay :)

It's actually very hard to "fail" the medical. If you don't have HIV, tuberculosis, or a severely debilitating life-altering disease or an infectious disease that is a serious threat to the public's health, you're fine. :yes: Run of the mill problems like high blood pressure or being overweight or what have you generally don't matter.

By the way, make sure not to lose the white sheet with the immunization information. That is your copy (there's another one inside the envelope) and you need it to complete adjustment of status once you're in the USA. And don't open the envelope or you'll void its contents. :P

If you do not plan on driving it right away you can bring it with you an import it later. That will give you some extra time to worry about all the paper work when you are not so rushed. You will need to get the compliance letter and import it before you will be able to register it.



That's great news! I won't be driving it in the winter months in any case, so that gives me a few more months to save up for it, and figure it all out. What with the wedding plans and needing money for the AOS and maybe covering a few months of mortgage till my house sells, every little bit helps. My biggest fear was having it confiscated at the border and no money to ship it home...

The compliance letter is the thing the guy at the dealer had no idea what to do about it. I have no idea where to go for that... I have an email into the company, and hopefully they can send me something, but as to being on letterhead, I'm hoping an email will do.

My interview and medical and eventual POE is fast approaching. Stuck with also having to sell my house, I've let slide the decision on whether or not to import my bike.

I own a 2007 Vespa LXV150cc and really would prefer to bring it with me, but I have several questions.
(I'm afraid looking online and even speaking to the local dealer were of no use, so I'm asking my favourite experts: YOU!)

#1 Can I just bring it with me and decide later on to import it if I so choose? (I know you can bring it for personal use up to a year, but if you don't import it immediately, do you lose the chance to later?) This would be my best option as I don't know the costs involved in moving it.

#2 I can't find any places listing my vehicle, I know it meets guidelines, but I don't know how to go about proving it. The dealer where I purchased it has closed down, and when I called the other local dealer, he had no idea how to help me. I haven't been able to reach anyone from Vespa Canada, and I've read you need to contact the manufacturer, so really? Italy?? What exactly is the form/letter I need from them anyway?

#3 How do I determine a value for my vehicle? I've looked at the Kelly bluebook, and it only lists the little e-bikes, and my bike is actually classed as a motorcycle. It's purchase price was 9000$, but no way do I want to use that... Do I just make something up??

#4 What exactly will they require from me at the border if I import it now, and if I import it later?

#5 What other questions am I not asking, like, do I also need to worry about importing the helmets? They meet DOT standards, but they are also manufactured in Italy, (MOMO)

I have a home still in Canada, and will need to keep my bank account active here to pay mortgage payments on until it sells. Of course, if I make any money on it, then I will have money to bring, but really? I'm looking more at bringing debt with me at this point! lol!!

I sold a home for more than my asking price because the parties involved ended up in a bidding war. I went with the one that had the least conditions and that I liked better. It was actually the lower offer, but only by a little, something like 500$ With the seller potentially being a company, it makes it harder to be the party that is liked more, but you can still beat them by putting no conditions on your offer, a quick closing date, and offering more than what the list price is.

It's entirely possible that the "bidders" are hoping to purchase the house really cheap for a fast flip, but if you purchase it closer to market value, it makes the house less profitable for the other bidders and you'll likely get it. Decide how badly you want this house. If it was priced 10k more, would you still be wanting it? What about 15 or 20k?

Don't break the bank to get it though, after all, at this point, it IS just a house, and you can always wait for another one to become your home. But if it truly IS a dream home, you need to figure out what the MOST you can comfortably afford is, and consider that price as your offer. You can gamble, and still bid low, (and maybe even win) but make sure that if you do that, you will be okay walking away for not raising your price a little.

Good luck, and I really hope that the story of the multiple bids is a little overblown... fingers crossed for you!

I emailed AND called Dr Seidens in Montreal and in Toronto, I didn't care where they booked me, I just wanted an appointment. I DID get a reply to the email, with a different number to call, and I actually got a live person there, that transferred me to the booking department, only to leave the same message with the same voicemail from the other number. I called close to a month ago, and left about 6 messages.

I finally broke down and called Medisys, and got a live person on the phone immediately, and he booked me an appointment.

Still haven't heard from Dr Seiden's group.... (either location)

and yes, I too live a lot closer to Toronto, but decided to make the most of it, and am having a week in Montreal with my mom!

I gave my employer notice that I might be giving notice after my interview. I have a really good relationship with them, and would feel bad only giving them two weeks notice when I've known for a year that I was going to be leaving them. This gives them time to interview people, and they know not to offer employment to them before I give them the go ahead. I also get to train my own replacement, something that I really appreciate being able to do, because I worry about leaving them in the lurch with no one to take over for me!

I would say that if your rapport with your boss is like mine (they're flying in to the US to come to my wedding in December even, shutting down the whole doctor's office!) then do what you can to help them make the transition smooth. I told them when I got my interview date. Until then, there really was no workaround date so I kept it quiet.

If your rapport is not like that, and you need to ensure that you have a job, in case of the worst case scenario, then yes, don't tell them until you have that visa in the bag!

(and if you hate them, phone them from the airport in the US after your plane lands and tell them you won't be in for work in the morning...)

I'm leaving on Thursday, but not driving. I'm taking the bus! There's a coach canada bus line from megabus.com that has LOADS of buses running between the two cities every day at all hours, and the price is incredible, something like 17$!!! My mom, my son and I booked round trip tickets for the three of us for 72$

Yes the train is faster.... but quite a bit more expensive, and let's face it, missing time off work when we already are spending so much to do the medical and interview, animals to the kennel, moving costs.... what's an extra couple of hours?? We just plan to bring a laptop with a movie on it, and a book!

My mother is a landed immigrant from England. It was difficult for her to get a passport, but she finally has one, so she can come to my wedding in December. She's been told by her sister that she needs a form filled in to be allowed to re-enter Canada. I was unaware that British subjects needed permission to enter Canada, and because she can't find a form, or information about a form, I thought I'd ask my favourite helpful people on here, hoping that someone here knows the answer. Last thing I want is my mother being stranded in the US, she's been living in Canada for 40 years!

I'm in Montreal at my hotel, Place des Arts, on Sherbrooke West. Place des Arts is right behind us and we can see all the lights from the balcony, it's really cool! The hotel room is really huge too, we have our own kitchen and a separate bedroom, really really nice. Ceilings have to be 12 feet high at least! We have our medical this afternoon, it's a 4 minute walk, so we're gonna head out and look around for a bit!

OMG, freaking out? YES!!! can't sleep, breaking out for the first time in YEARS, not sleeping (did I mention that already?) and can't for the life of me find my fu@*ing long form birth certificate for my son and his dad's REAL death certificate!!! I DO have the normal birth certificate and the death certificate from the funeral home but need to turn my house upside down to find those forms!!!

OMG, I don't think I have the entire copy of my fiance's passport, tbh, I don't think I have the photocopy of his info page.... gotta check.... (OCD much anyone??)

I have a really really big pile of papers to bring with me. It's humongous. Ridiculous, ginormous. What can I say??? Facebook chat emails 4 pages to one sheet of paper... still too many pages. 11 months of itemized phone calls. (thank god BELL has a 20$ per month unlimited North America plan or I'd be in debtor's prison, @ an average of 2-3k worth of phone calls per month.... um, yeah.... proof of a relationship??

13 photos of us together in various places and with friends and family. Plane stubs. Confirmation of moving truck rental. Confirmation of wedding venue booking. Copy of wedding invitations. copy of wedding ring receipt. (getting married? you BET CHA!!!)

okay, so, did I mention I was freaking out??
