IR-1 / CR-1 Spouse Visa Process & Procedures	I got the permanent resident card	Did you fill out the forms I-485,I-765 in your married name or your maiden name?If you use maiden name in all the forms then that will be show on your greencard. My visa were in maiden name too because I entered US by K1 and also my passport.But when I filled out all the forms,I used my married name and also I submitted copy of marriage certificate. Hi rheanick,yes you have to report your marriage first here in the U.S. before you can amend your passport to your married name.Anyway you can do it both at the same time while you are here in the U.S. but if you plan to renew it in the Philippines,you have to report your marriage first here in the U.S. It happened to my friend..she went to the Philippines last February for her vacation and her passport was about to expire so she decided to renewed it there but she thought she can renew it using her married name..the DFA told her she can renew it but still using her maiden name because she didnt report her marriage yet so the DFA told her as soon that she get back here in U.S. report her marriage at the nearest Philippine Consulate and wait a mail from the DFA that they received it and that her marriage has been reported. Then after 2 months when I had the HPV #2,I had the last shot which is HPV #3.Because it was written on the record that I need to have the last shot after 2months which is the time frame and since it's appropriate on my age,why not have the last shot so I had the completed HPV vaccines,and it's good also to avoid cervical cancer. Philippines	Anyone similar experience with Social Security Number	I arrived here in the US January 18,2009. Applied SSN using maiden name January 27,The lady told me to just come back after we got married and bring MC to change it to married name.
