A notarized copy of your parent's consent. 8. P250 for seminar 9. P400 for CFO sticker P250 for the seminar at SMEF. Andafter the seminar P400 for the sticker and certificate sa CFO. CFO office is located beside DFA Cebu. Malapit sa Plaza Independencia. Hi cooky, where will you attend seminar? If you will attend in Cebu (where I attended) you no longer need to bring 2x2 pics. They are the ones who will take your pics thru PC (just like when you get NBI clearance). As long as you already have your certificate, CFO should stamp your passport (with the visa) with their sticker. I cannot speak for CFO Manila, sorry Good luck, hope you will get your visa soon!
