One of the reasons its 90 days (rather than a shorter period) is to give you time to live together and make sure marriage is still what you want (its hard to judge these things in a long distance relationship). Sometimes it takes being together in a more normal life setting to realize one does or does not want to marry. If your fiance is not ready to marry, then you need to return before the 90 days, or you risk having problems with possible future travel or immigration to the US.
